All House League and Provincial teams travelling off-Island are required to complete and submit a travel sanction request to RPEI, 30 days prior to departure.
Incident Report Form
This form should be filled out by clubs to document any accidents which occur during sanctioned events. If either of the parties involved makes an insurance claim, the accident claim form needs to be submitted as well.
Accident Claim Form
This form is to be filled out by anyone who needs to make an insurance claim. It should be submitted to RPEI within 15 days of the accident in order to make a claim.
All completed documents can be sent to the Ringette PEI office at ringettepei@gmail.com to be submitted to the insurance company.
Expense Claim Form
Ringette PEI will reimburse legitimate travel and meal expenses at the following rates:
Mileage - .50 cents/km
Breakfast - $10, Lunch - $15, Supper - $20
Reimbursement will also be given for expenses incurred for Come Try Ringette and other events or programs under Ringette PEI. The expense claim form must be filled out and all receipts attached for purchases.
